These Terms of Service (the "Agreement") are a legally binding contract between C&L Venture Group, LLC, doing business as Elevate Integrated Health ("Company," "we," "us," or "our"), and you ("User," "you," or "your") governing your use of our website at https://elevateintegratedhealthtn.com, our services, and our SMS messaging program. By accessing or using our website or services, you agree to be bound by these Terms. If you do not agree, you must discontinue use.
You must be at least eighteen (18) years of age to use our website or services or to participate in our SMS messaging program. By opting in, you represent that you meet this requirement and that you are the account holder or authorized user of the mobile number provided.
The Elevate Integrated Health SMS Program is operated by C&L Venture Group, LLC, doing business as Elevate Integrated Health. The program has two separate, optional parts: (a) non-marketing (transactional) messages such as appointment reminders, confirmations, and service-related updates; and (b) marketing messages such as promotions, special offers, and announcements. You may join either, both, or neither.
You may opt in through our web form at https://elevateintegratedhealthtn.com by entering your mobile number and checking one or both optional consent checkboxes, which are unchecked by default. The form can be submitted without checking either box. Providing a phone number alone does not constitute consent to receive text messages.
Consent to receive text messages is not a condition of any purchase or service.
Message frequency varies. Marketing: up to 10 messages per month. Non-marketing (transactional): up to 6 messages per month. Msg & data rates may apply. You are responsible for any fees charged by your mobile carrier under your plan.
You may cancel at any time by replying STOP to any message. You will receive one confirmation message and no further messages will be sent. To rejoin, reply START or opt in again through our website.

Wireless carriers are not liable for delayed or undelivered messages.
Our text messages will not include sensitive health information.
Your participation in the SMS program is subject to our Privacy Policy (https://elevateintegratedhealthtn.com/privacy-policy). No mobile information will be shared with third parties/affiliates for marketing/promotional purposes.
The Company grants you a limited, non-exclusive, non-transferable, revocable license to use the website for personal, non-commercial purposes.
You shall not:
a) Copy, modify, distribute, or transmit any website content without prior written consent;
b) Use the website for any unlawful or fraudulent activities;
c) Engage in hacking, data mining, or unauthorized access to Company systems;
d) Attempt to decompile, reverse-engineer, or replicate any proprietary software.
